Job description

Schedule: Monday-Thursday 8am-5pm Pay Range: $90-95k per year: Essential Duties: **This role will see clients at 4 different offices and travel between offices will be required. - Provide exceptional behavioral health interventions, within an integrated care setting, utilizing therapeutic intervention models (brief solution focused, CBT, MI, ACT etc.) - Experience and willingness to work with a diverse population with a wide range of mental health challenges, including substance abuse. - Work effectively and collaboratively with the primary care medical providers to deliver whole person care - Coordinate care with other community providers and provide linkage and referrals to other services as needed. - Set and monitor achievable clinical goals and outcomes - Participation in provider team meetings. - Ability to provide both in person and telehealth treatment. - Willingness to explore other clinical interventions that may benefit the patients of the practice (i.e., clinical groups or other modes of providing care as deemed appropriate and beneficial) - Timely documentation - Other duties as assigned Qualifications: - Master's Degree (from accredited university/college) in Social Wo...
