As an Enrollment Case Manager, you’ll play a vital role in delivering compassionate, person-centered service and support individuals to enroll in Medicaid Waiver and State General Fund services. Your days will be filled with: Conducting in-home visits to complete assessments, connect individuals to services, and facilitate service plan meetings. Engaging with families and providers in the community to build trust and foster strong partnerships. Collaborating at our Aurora office, sharing insights and strategies with your team and other departments to enhance service delivery. Working from home, which includes hosting virtual Zoom meetings with individuals and their teams, documenting and coordinating services, and participating in virtual office hours, team meetings, and more.
