Care Dimensions Healthcare is currently seeking a compassionate, community-centered, and motivated Full-Time Community Health Worker (CHW) to join our growing team. This position will report to our Corona office and primarily support patients throughout the Hemet area. This is a full-time Monday through Friday position with an hourly pay range of $25-$30 per hour, depending on experience.
