Extended Home Care of NY is looking for CFY, Speech Language Pathologists to conduct home visits in NYC, under the supervision of a licensed therapist. The speech therapist contracted or employed through the organization provides services to patients according to a written physician's plan of care. This may include but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: Assesses patient’s speech and language abilities and deficits. Prepares an initial assessment of the patient’s condition and perform periodic reassessments. Develops a plan of care in accordance with the authorized practitioner’s plan of treatment for therapy services and according to agency policies.
