Employment Type: Full time Shift: Day Shift Description: The St. Joseph Mercy Hospital Pharmacy Practice Residency Program is an organized, directed, postgraduate program intended to develop a highly trained pharmacist with advanced knowledge and skills in the application of drugs and drug therapy to pharmacy practice. The Pharmacy Practice Resident will perform all staff pharmacist functions on a regularly assigned basis. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: Performs Staff Pharmacist functions on a rotating basis. Drug therapy monitoring of assigned patients and communicating relevant findings and recommendations to other health care professionals who are also responsible for the patient's care and documentation of outcomes. Development of patient-specific drug therapy management plans and provision of oral and written consultations with other health care professionals regarding drug therapy selection and management. Patient education and counseling regarding drug therapy and drug-related disease prevention. Preparation of medication histories for patient's, for the patient's medical record or other databases.
