This position is 40 hours per week, 8:00 AM-4:30 PM M-F (tentative) Responsibilities: - Performs duties related to the medication reconciliation process. Includes patient interview, home regimen research and electronic health record documentation. Works with and directs pharmacy technicians dedicated to the medication reconciliation process. - Provides pharmaceutical care (PC) through drug therapy assessment, planning, implementation and monitoring that is consistent with established standards of practice.
