Job description

Position Summary: The MCW (Medi-Cal Waiver) Program Coordinator will be responsible for fulfilling a broad range of analytical, planning, engagement, and implementation functions of the MCW which is focused on improving the health of most vulnerable patients. The MCW Program Coordinator, under the guidance of the Program Supervisor, will assist with program coordination during the startup phase through implementation of the MCW and will serve as a liaison for the care coordination work and activities being provided by the MCW staff across internal and external organizations. Primary duties shall include performing research and data analysis, reporting, operational and conceptual problem-solving, building partnerships, and coordinating patient care activities.
