Our mission statement definitely speaks to what La Clinica de Familia stands for, which is to empower and enrich families, individuals, and communities by providing quality medical, dental, behavioral health and educational service for people of all cultures. $15.35 to $20.59 Non-Exempt Job Summary: Under the direct supervision of the Clinical Team Lead, the Patient Navigator–Community Health Worker plays a key role in supporting clients as they navigate behavioral health, physical health, and social service systems. The Patient Navigator will focus on patient and community engagement, outreach, care coordination, and advocacy. Ensuring individuals receive timely, culturally responsive and person-centered services. The Patient Navigator will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness.
