Cobble Hill is currently seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This is a confidential search for a professional who is passionate about advocating for residents, supporting families, and helping ensure the highest quality of life for those in our care. Position Summary: The Social Worker is responsible for supporting residents' psychosocial well-being, assisting with care planning, coordinating discharge planning, and serving as a liaison between residents, families, and the care team. The ideal candidate is empathetic, organized, and experienced in skilled nursing or long-term care settings. While LMSW licensure is not required, candidates who hold an LMSW are strongly preferred.
