Here’s what you’ll be doing: - Provides clinical services within the scope of a Family Practice or Pediatric Nurse Practitioner, serving the general community, but with a special focus on Pediatrics - Diagnoses, treats and provides high quality patient centered care for medical conditions in children and adults following accepted medical standards of practice - Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions - Refers patients to specialists and to relevant patient care components as appropriate - Evaluates and revises patient care/treatment plan with patient and family as necessary to achieve improved health - Provides education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment - Provides accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicates them to other members of the health care team; maintain accurate, legible, timely and confidential records - Participates actively in the advancement of FCP’s Medical Home model, including innovative ideas, function as leader of a...
