Description Summary: The Community Habilitation Coordinator is responsible for coordinating, monitoring, and supporting services that help individuals with intellectual and developmental disabilities develop skills for independence and community integration. The coordinator ensures services are person-centered, compliant with OPWDD regulations, and aligned with each individual’s Life Plan.
