The BHCC/ECM Case Manager will assist providers with complex patients and offer general support when situations arise. This position will require both medical floor and field case management. This position will require immediate patient assistance on the medical floor. The BHCC/ECM Case Manager will manage a case load of complex care clients. All clients may be engaging in behaviors that pose health and safety risks. A successful BHCC/ECM Case Manager will meet individuals “where they are at” and provide non-judgmental, non-coercive services, and resources to individuals seeking help.
