Job description

Position Overview : The Therapist plans, directs, and coordinates treatment for patients in inpatient and/or outpatient SUD and mental health programs. Inspires and motivates patients to help them develop tools to work toward recovery using evidence-based and person-centered practices. Delivers individual and group therapy as well as family coaching and support. Coordinates care with the multidisciplinary treatment team. Specific Responsibilities: Evaluate patient' mental and physical health, substance use, or needs and assess their readiness for treatment Work with patients and the multidisciplinary team to develop treatment goals, documented in a treatment plan Utilize Measurement-Based Care (MBC) screeners to assess patients, integrates results into treatment planning, and leverages data to track progress and influence care decisions. Review and recommend treatment options with patients and their families that support full continuum of care.
