This position is eligible for a prorated $2,000 retention bonus. Job Summary: The Providence Center Residential Case Manager Grandview Apartments will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible. Schedule: Sunday 3:00pm-11:00pm, Monday 3:00pm-11:00pm, Tuesday 3:00pm-11:00pm, Saturday 7:00am-3:00pm. 30 minute unpaid lunch/break per shift. 2nd shift, 3rd shift, weekends, and holidays eligible for shift differential compensation. Duties and Responsibilities: Initiate treatment and advocate for consumer within network of care. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
