Schedule : Monday-Friday, day shift The Health Information Representative is responsible for creation and review of Plans of Care to ensure CMS program compliance is met. This position handles technical reviews, audits and maintenance of Home & Community Care patient records. This role works collaboratively with the clinician and provider to ensure certifications, orders and clinical documentation throughout the patients' care have been completed and medical record issues that impact the revenue cycle process are resolved in a timely manner.
