Community Nurse - EN (Full-time or Part-time)

Job description

We are looking for a passionate and collaborative (full-time or part-time) Enrolled Nurse to join our small, dynamic Clinical Team. Reporting to the Quality and Clinical Manager, this role will provide care to our clients in their homes, enabling them to live their best lives. Whilst this is an office-based role, you will regularly meet with clients in their own home and under supervision from clinicians, assess their needs and implement a preventative based approach to health care through clinical skill and knowledge. Key responsibilities of the role include: - Under supervision, conducting clinical assessments of clients in their homes - Identifying clinical care needs and taking appropriate action - Liaising with client’s care team (family; GP; allied health service providers) to ensure a coordinated approach to client care. - Initiating appropriate coaching with clients for their self-care needs - Conducting training for Support Workers for their clinical skill development - Performing nursing care with clients with complex needs - Review of clinical incident management and report outcomes and recommendations to management.
