Coleman Care Centre, located in Barrie, Ontario has an exciting opportunity for a REGISTERED DIETITIAN to join their LONG-TERM CARE village team. This a contract position for a total of 47 hours monthly in Long Term Care . At Schlegel Villages you can be YOU while being part of OUR Family. To be successful as our new Registered Dietitian you will: - Complete Resident Nutrition Risk Assessments for all new admissions - Monitor and follow residents who are identified at high nutritional risk - Determine person responsible for monitoring residents at moderate and low nutritional risk - Recommend specific nutritional interventions and provide information to the Care Team about implementing recommendations, monitoring and follow-up as required - Collaborate with the resident’s attending physician regarding the nutritional plan of care and associated assessment needs, i.e.: lab results, vitamin supplementation, diet changes, etc. - Participate as a member of the Interdisciplinary Team in the development and evaluation of nutrition care plans which includes an assessment, resident centred goals and nutritional care plan to meet the individual needs of the resident and evaluate the outcome...
