Job Purpose: The Licensed Clinical Social Worker is responsible for coordinating and implementing appropriate methods of treatment to patients in the behavioral health program. DUTIES INCLUDE, BUT ARE NOT LIMITED TO THE FOLLOWING: · Greets patients and introduces self. · Presents a professional image and customer focus. · Verifies patient information, medical history, medications, and confirms purpose of visit. · Conducts pre-surgical evaluations and diagnosis as required for surgical implant procedures · Provides assessment, diagnosis and individual therapy treatment to patients in the behavioral health program. · Monitors the effectiveness of interventions. · Performs routine and crisis counseling/interventions, and suicide risk assessments as needed. · Provides referrals for services as needed. · Responsible for the conten...
