Hospice RN Case Manager RN — Where Support Meets Impact Office Location: N168W22730 Prairie View Ln, Jackson, WI 53037 Coverage area: Travel throughout the territory is required- Primarily Washington, Ozaukeend northeast Milwaukee Counties - Lower patient-to-nurse ratios for meaningful, unrushed hospice care - Strong interdisciplinary team support—collaborative care with Social Work, Chaplaincy, Aides, and providers, so you’re never practicing alone - Leadership that understands hospice nursing - Dedicated internal after-hours triage team to support nurses outside of regular business hours - Homecare Homebase (HCHB) EMR for streamlined documentation Schedule: Monday-Friday 8AM-4:30PM: Are you a Registered Nurse looking for a new opportunity?
