Schedule: 20 hours per week (Monday- Friday) Position Summary: The Patient Health Information Coordinator – PACE External Records, is responsible for obtaining, tracking, and maintaining outside medical records for PACE participants to ensure a complete, accurate, and timely health record. This role supports interdisciplinary care planning, regulatory compliance, and continuity of care by coordinating the receipt of records from hospitals, providers, facilities, and other external entities.
