Description This is for a Part Time PM Shift. Through a holistic approach, high quality comprehensive care is provided timely and efficiently to promote a positive end of life experience in the PCU. Requirements: ESSENTIAL DUTIES & RESPONSIBILITIES: · Conduct admission assessments · Establish plan of care in coordination with interdisciplinary team approach to collaborate, revise, update, and assure services are provided according to established plan · Make assignments and supervise patient care provided by volunteers and staff members · Ensure physicians, families, and appropriate staff members are promptly notified of changes in patient condition · Make referrals to other services as needed in coordination with social workers · Educate families and patients on disease processes, dying process, treatment plan and assist them in making informed choices about care promoting independence · Provide active listening and support to patients and families through the trajectory of their disease progression · Provide services at the time of death, including notification of staff per policy · Provide medications timely and accurately per physician’s orders · Ensure safety of all residents i...
