Assisted Living Registered Nurse (RN Case Manager)

Job description

FlexStaff is seeking a Registered Nurse (RN) to work full-time as a Nurse Case Manager for an Assisted Living facility in Queens, NY. In this role, you’ll lead clinical assessments, coordinate individualized care plans, and serve as a critical liaison among residents, families, providers, and staff to promote safety, well-being, and independence. Schedule: Monday -Friday 7:30A- 4:00P, day shift, with occasional early evening or weekend coverage for urgent needs. Sign-on bonus $15,000. Qualifications: Current, unencumbered New York State Registered Nurse (RN) license required. 2+ years of clinical experience in assisted living, long-term care, home care, case management, or geriatric settings (ALP experience a plus). Strong care coordination skills with knowledge of community resources, payer requirements, and transitions of care. Familiarity with NYS DOH regulations; experience with UAS-NY assessments and/or managed long-term care (MLTC) preferred. Proficiency with EMR systems and strong documentation skills.
