Job Description The Fire Department, City of New York (FDNY), seeks a full-time Case Management Nurse in the Bureau of Health Services. Reporting directly to the BHS SUPERVISOR OF NURSES the successful candidate will maintain a comprehensive case management system for the BHS for Medical Qualification of Firefighter and EMS Candidates, Annual Medicals and BHS Clinic for FDNY incumbents. The candidate will use clinical tools, data and other information to complete assessments of patient needs. Create individualized and comprehensive case management and treatment plans. Determine service-connected coverage eligibility for various treatments and treatment programs. Clearly explain the reason for prescribed treatments and treatment plans to patients. Carry out adept nursing assessment of patient health status. Educate patients on benefits, services and other resources. Communicate with patients, physicians and insurance companies to secure successful outcomes. Document complex medical information in our electronic medical record (EMR) to outline and improve health results and determination of eligibility of candidates.
