ABOUT THE JOB: The RN Care Manager provides patient-centered clinical care management for hospitalized patients in inpatient and emergency department settings. Serving as an integral member of the interdisciplinary healthcare team, this role conducts comprehensive assessments, develops and implements care plans, and coordinates interventions to support safe, timely, and effective transitions of care. The RN Care Manager identifies barriers to progression, facilitates discharge planning, and applies evidence-based practices to promote quality outcomes and the clinically appropriate, cost-effective utilization of healthcare resources. This is a full-time (40 hours/week) position. Responsibilities: - Identifies and prioritizes high-risk patients to support timely, effective care transitions. - Conducts comprehensive, patient centered assessments to identify gaps in care and implement appropriate interventions, education, referrals, and consultative services - Provides clinical care coordination to diverse patient populations, utilizing evidence-based practices to develop and adjust transition plans based on clinical needs, available resources, and patient preferences, including cultur...
