About the Role: The Primary Health Team Leader provides operational, clinical and professional leadership to the Primary Health team and is responsible for the coordination and delivery of multidisciplinary community and primary health services. The role supports the health and wellbeing of the local community through the provision of accessible, integrated and person-centred care.
