Job Details Under the supervision of the Ambulatory Pharmacist Program Medical Director provides Population Health Ambulatory Pharmacy focused care services to Jefferson Health Providers and patients. Will collaborate with central and local level population health teams to develop and implement strategies that support the high value, high quality, patient centered medical care in Jefferson Health Primary Care and Specialists Practices. Patients serviced range in age from 0 to 100+. Job Description: ESSENTIAL FUNCTIONS: - Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. - Develops and implements pharmacy initiatives directed at improving prescribing patterns of Jefferson ambulatory providers that promote high value, high quality, safe and cost-efficient prescribing. This may include identification of prescribing patterns that may lead to improper drug selection, sub-therapeutic dosage, overdose, patient failure to receive medication, adverse drug reactions, drug interactions, and medication without indication and unnecessary prescribing of high-cost medications. - Develops and implements patient education initiatives related to the indivi...
