About the role: The Senior Case Manager is responsible for the provision of case management and support services to young people living in residential homes and will also participate in providing specialist services to young people aged 12 to 19 years old in MacKillop’s OoHC program and after leaving care for a maximum of 12 months. The Senior Case Manager will, provide outreach services to MacKillop’s OOHC Clients after leaving care, develop and implement relevant programs according to client needs, undertake all outlined duties expected in case management, including but not limited to: end to end case management of clients, accurate and timely case notes in line with MacKillop expectations, participation in associated meetings and provision of individual support and guidance to Young People, develop and support leaving care arrangements, provide expert advice to the case management team regarding complex cases, provide reports on progress of programs, including recommendations and under general and limited direction, undertake a variety of tasks as delegated by the Case Management Coordinator This is a Full-Time opportunity based in Forest Hill, VIC within our Out of Home Care Pr...
