Employment Type: Full time Shift: Description: Responsible for scheduling clients Home Health/Hospice services with clinician continuity. Identifies client need and schedule clinicians with the appropriate competencies to match client level of care. The Scheduler is expected to exercise judgement in determining the most effective and efficient method of scheduling and rescheduling patients. Provide optimal levels of communication with patients, clinicians, clinical managers, physicians, and homecare coordinators. May also provide coverage for multiple sites and/or Hospice support. Schedule: Monday - Friday - 8am - 5pm Provide weekend and holiday coverage on a rotating basis Position Purpose: The Scheduler plays a vital role in supporting timely, compassionate Home Health care. In this position, you will coordinate patient schedules with a focus on clinician continuity, ensuring that each individual receives care from the most appropriately skilled team members.
