GENERAL DESCRIPTION: This position is located at a Permanent Supportive Housing (PSH) program that provides affordable and stable housing to people who experienced chronic homelessness . The person in this position will meet with residents one-on-one to perform needs assessments, develop support plans, and identify resources that will help support each resident based on assessments. The Sequoia Crossing Case Manager will coordinate and maintain a healthy relationship with other community providers in order to ensure each resident gains access to the services that they need. MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required EDUCATION and/or EXPERIENCE - High School Diploma or GED and 2 years of social service experience. Equivalent combination of education and experience may be accepted. - Knowledge of/or experience with: the principles of coordinated assessment, working with unsheltered or vulnerable populations, as well as best practices in homeless service delivery is preferred.
