Employment Type: Full time Shift: 12 Hour Day Shift Description: Position Purpose At Saint Francis Hospital , the Emergency Department Registered Nurse Case Manager plays a critical role in coordinating patient care in a fast‑paced emergency setting. This role focuses on early identification of patient needs, care coordination, utilization management, and safe transitions of care to ensure high‑quality, cost‑effective outcomes while supporting patient flow and throughput from the Emergency Department. What You Will Do: As an Emergency Department RN Case Manager , you will serve as a key partner to ED physicians, nurses, and interdisciplinary teams to support timely, appropriate care decisions. Responsibilities include: Collaborating with ED providers and care teams to assess patient needs upon presentation Identifying appropriate levels of care and facilitating admissions, transfers, or safe discharges Coordinating care across inpatient, observation, and post‑acute settings Supporting patient flow, throughput, and length‑of‑stay initiatives Conducting clinical, psychosocial, and discharge planning assessments Applying nationally recognized medical necessity and utilization criteria...
