License Clinical Social Worker - Addiction Medicine (Full Time, Day)

Job description

At NorthBay Health the LCSW provides direct clinical social work service, counseling, and case management services to Addiction Medicine patients including psychosocial assessment, development of a plan of care, implementation and documentation of emotional, psychological, social, social, social, financial, environmental, environmental or follow-up referral needs Works collaboratively with Addiction Medicine interdisciplinary members and other colleagues within the NorthBay Health. Assesses patients and develops a psychosocial plan of care within established timeline. Responds to referrals in a timely manner. Monitors progress toward goals of plan of care. Facilitates integration with community-based resources to optimize patient health. Collaborates with community partners to optimize patient health. Documents assessment and interventions in patient’s medical record. Activities are consistent with System mission and values and in compliance with applicable regulations and System policies and procedures.
