Employment Type: Full time Shift: Description: Position Purpose The Care Coordinator is responsible for coordinating services, support, and resources to ensure individuals receive high‑quality, person‑centered care. This role serves as a key liaison among individuals, internal teams, and external providers to promote continuity of care, safety, and positive outcomes.
