As a Clinical Social Worker, you will provide compassionate social work services, clinical interventions, and complex discharge planning for individuals with a wide range of behavioral health needs. Working closely with an interdisciplinary care team, you will help design and implement comprehensive treatment plans that promote ongoing health, stability, and recovery after discharge. Essential Functions: Complete psychosocial assessments in alignment with departmental and professional standards to identify emotional, social, and environmental needs related to mental health conditions, illness, and life circumstances.
