Major Responsibilities: - Case management of sub-specialty, pediatric patient populations for coordination of care. - Plans, monitors, and coordinates individual patient care plan. - Utilize critical pathways and best practice guidelines as appropriate for both inpatient and outpatient needs. - Collaborates with the patient, family, specialist, pediatrician, and multi-disciplinary care team to assure appropriate, safe, cost effective, timely, and efficient care. - Organizes, educates, and schedules (if needed) pediatric, sub-specialty procedures, appointments, nurse visits etc. - Coordinates patient care conferences and educational offerings - Acts as the primary point of contact for patients and their families assisting with all compacity of care, triage concerns, medication education, medication refills, and all other requests within scope. - Particip...
