Home Care Coordinator - MA Department: Home & CBS: Employment Type: Full Time: Location: Holland, MI: Reporting To: Tami Loyd: Compensation: $20.81 - $26.37 / hour Description: Schedule: Full Time | 40 hours per week | Monday - Friday | 8:00 am - 4:30 pm Department: Participant Care | Holland, MI What You'll Do In This Role: Be the link that brings care home. As a Home Care Coordinator at LifeCircles, you’ll partner with the interdisciplinary team to assess needs and coordinate essential in-home services that help participants live safely and independently. From arranging durable medical equipment and supplies to organizing personal care, homemaking, respite, and community living supports, you’ll ensure services are delivered efficiently and seamlessly. Your attention to detail and collaborative approach will help create personalized care experiences that truly make a difference in the lives of those we serve.
