Job Title: Licensed Clinical Social Worker Department: Integrated Care: Location: RRH - 510 Kreag Road Ste 2 Hours Per Week: 40 hours per week SUMMARY: The Licensed Clinical Social Worker provides Mental Health/Chemical Dependency diagnostic assessment, treatment planning, and evidence-based interventions for people who have mental illness and addiction. Treatment is provided in the context of a multidisciplinary clinical team, supervised by a physician. Leadership and/or supervisory duties as delegated by manager. Provide superior customer service by modeling the Rochester Regional Core Values and professional conduct guided by the NASW Social Work Code of Ethics. RESPONSIBILITIES: Provides initial and ongoing assessment, Evidence-Based clinical interventions to a designated caseload. Develops Treatment Plans that are individualized and culturally sensitive Provides individual, group, and family psychotherapy. Participates functionally as a member of a multidisciplinary team in coordination and delivery of patient care.
