Description LOCATION: Wayne County Healthy Communities: Peace Baptist Church - 13450 Goddard St. Hamtramck, MI 48212 POSITION SUMMARY: The Community Health Medical Assistant supports the mission of the Neighborhood Wellness Center by providing high-quality patient care, preventive health screenings, and health education in both clinic and community-based settings. Working under the supervision of the Community Health Nurse and providers, the CMA plays a key role in patient engagement, clinical workflow, and outreach activities designed to address chronic disease prevention and improve health equity in underserved populations. ESSENTIAL FUNCTIONS: Clinical Support & Screenings: - Perform preventive health screenings (e.g., blood pressure, blood glucose) - Collect and record vital signs, and intake information accurately. - Support immunization and vaccination workflows under RN/provider supervision. - Assist in point-of-care testing (POCT) and specimen collection. Care Coordination & Outreach: - Participate in community-based outreach events (e.g. health fairs, school screenings, church/wellness events). - Assist the Community Health RN & CHWs in connecting patients to medical, beha...
