Work Schedule: 100% FTE; Day & Overnight Shift Rotation. Overnight shift Monday – Sunday (9:00PM – 7:00AM) and AM Shift Monday – Friday (7:00AM – 3:30PM). Hours may vary based on the operational needs of the department. You will work at University Hospital in Madison, WI. We are seeking a Clinical Pharmacist to: - Provide comprehensive medication therapy management as a key member of the interdisciplinary inpatient care team, ensuring drug therapy is safe, effective, efficient, and cost effective. - Perform a full scope of clinical and distributive pharmacy services, including participation in patient care rounds, medication reconciliation, medication therapy monitoring, medication order review and management, and patient education. - Deliver high quality pharmaceutical care to a diverse critical care patient population, including cardiovascular and medical surgical patients with complex medication needs.
