Position Summary: The Systems Integration Facilitator will provide interagency team meeting (ITM) facilitation and leadership that is comprehensive, highly skilled, objective and neutral, trauma-informed, and person-centered for assigned children, adolescents, and transition age youth with complex needs within the Bucks County Behavioral Health system. For all levels of care, planning is essential to keeping families informed and connected serving as a mechanism for supporting the professionals in working together with families in a solution-based manner. Facilitation assignments will be referred by the County and are intended to assist a youth, their family and team address a lack of progress or barriers to planning on a temporary basis. Responsibilities: - The Systems Integration Facilitator (SIF) will be the lead organizer and facilitator of the interagency team meeting (ITM) process for assigned youth up to age 21 and their families within the program. - Engage families, youth, and system partners in exploring strengths, needs and barriers that impact the youth. - Receive, review and determine program eligibility for new referrals. - Facilitate an ITM planning process that is s...
