We are looking for a licensed MSW or RN Care Manager to service both seniors and adults with physical disabilities, mental health issues, dementia, and traumatic brain injuries. Essential Job Responsibilities:  Assessment of new patients and maintain ongoing patient evaluations.  Create individualized care plans based on specific patient needs.  Respond and handle patient crises in a professional and timely manner.  Develop coherent, legible, and grammatically correct assessments, care plans, and monthly reports on a timely basis.  Document all patient interactions in our online software program.  Represent the organization through positive interactions in the community.
