Work Shift: Day (United States of America) Job Description: Wellstar Advanced Heart Failure & Palliative Care is a hospital outpatient department of Kennestone Hospital. Our goals are to improve clinical outcomes and quality of life for patients with heart failure while improving patient understanding and self-management of their disease process. Our care is based upon current evidence and best practice, and teamwork and collaboration are at the core of our care model. Our clinic operates Monday-Friday and is closed on weekends and major holidays.
