Care Coordinator Griffith, NSW | Permanent Full-Time: Summary: The Care Coordinator plays a key role in supporting the delivery of safe, person-centred care within the residential Aged Care environment at The Pioneers Lodge. Working collaboratively with Registered Nurses, clinical leaders, care staff and allied health professionals, the Care Coordinator assists in coordinating day-to-day care operations, supporting resident wellbeing, facilitating communication, and promoting continuity of care across the home.
