RN - Transplant Coordinator

Job description

Provides coordination/case management for the transplant related activities of patient caseload across the continuum of care – pre-transplant, through post-transplant. Provides patient/family education, coordinates patient evaluation, maintains waiting list registration and maintains patient readiness for deceased donor transplants. Arranges transplant admission, monitors clinical pathway progression and provides post discharge follow-up. The coordinator provides professional/peer education, and participates in the development of patient education programs. The coordinator prepares for, and participates in outreach clinics, with physicians. Participates in recipient selection for placement on the UNOS wait list as part of the transplant committee, then communicates the decision to patient, patients' nephrologist, and dialysis program. Provides inpatient and outpatient clinic transplant education. In the post-transplant patient clinic provides ongoing education, monitoring of labs, and arranges outpatient infusion and radiologic procedures, and any other patient needs.
