Job description

JOB DESCRIPTION OVERVIEW: This individual's responsibility will be to design, develop, implement, supervise, and provide specialized training and services to clients in the program. This person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services and resources available and necessary to them. These services and resources may include education and employment, housing, recovery and prevention services, and aftercare supports. Duties may include, but not be limited to providing intake services to clients; preparing and making reports, recommending continuing care plans, assisting other staff members in the performance of a variety of difficult and varied work duties such as planning vocational/employment activity sessions, conducting workshops for skill building, resume writing, computer literacy, etc. and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals and aftercare plans.
