Heartland Home Health is seeking a PRN Physical Therapist to provide care to our patients in our Midland, MI branch. What You’ll Do as a Home Health Physical Therapist: - Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. - Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function. - Adjust treatment plans and techniques as needed to achieve optimal outcomes. - Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care. - Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices. - Document all visits, assessments, and progress notes...
