Description ABOUT FAMILY HEALTH CLINICS: Family Health Clinics provide team-based, comprehensive longitudinal primary care services to patients in the area. Our communal goal is to attach, ensure access and provide comprehensive medical care to patients living in the local community. We have exciting opportunities for medical staff and health care professionals to join our Family Health Clinic team. JOB SUMMARY: Within the context of a team-based model of care and in collaboration with the Primary Care Provider (Family Physician or Nurse Practitioner) and other members of the PCN Interdisciplinary team, the Primary Care Social Worker assesses, plans, implements and evaluates social work care for patients/clients and caregiver(s).
