Job description

The Family Nurse Practitioner provides quality, comprehensive primary health care services to the Center’s Rheumatology patients consulting with supervising physician and abiding by appropriate clinical protocols. Reports to the Director of Specialty Care related to the delivery of medical care for Rheumatology patients. DUTIES AND RESPONSIBILITIES: 1. Examines, diagnoses, treats and/or refers Rheumatology patients assigned to physician’s panel, on an out-patient basis. Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assess medical conditions consulting with supervising physician. 2. Maintains electronic health records (EHR) for each patient seen in accordance with the Center’s established Health Information Management policies and procedures providing appropriate, legible documentation. 3. Assures continuity of care by providing medical services on a timely basis as per Clinic schedule within the Center’s scope of practice. 4. Refers clients for appropriate specialty care services, lab and imaging studies and any other ancillary services that are appropriate for patient’s management and care consulting with supervising physician as needed. 5...
