POSITION SUMMARY: Reporting to the Manager Department of Integrated Discharge Services, the registered social worker will work to complete weekly mandatory ALC reports for the hospital as directed by the province. QUALIFICATIONS: Bachelor of Social Work degree from an accredited University required Master’s of Social Work degree from an accredited University preferred Current registration with the Ontario College of Social Workers and Social Service Workers Minimum of 5 years frontline social work in a hospital setting preferred Experience utilizing computer software such as word, outlook, excel, power point, WTIS ALC IS, and other computer systems to view data, create and submit reports Experience working within a hospital setting preferred Knowledge of community resources preferred Strong communication skills required Ability to maintain consistency with the mission and values of St. Joseph’s Healthcare Hamilton Strong attention to detail, multi-tasking and prioritizing when responding to requests for reports Ability to shift attention and priorities based on demands of department and hospital RESPONSIBILITIES: Maintains confidentiality and release of information in accordance wi...
