Company: Providence at Home with Compassus Position Summary: ***Spokane based supporting Providence Sacred Heart and Holy Family Medical Centers*** The Care liaison Manager is a leader who connects hospitals with the Home Health and Hospice teams, making sure care is coordinated, relationships stay strong, staff are supported, and value-based care goals are met. In connection with the JV's Value-Based Enterprise (VBE) care coordination agreement with Providence hospital systems, the Care Liaison Manager will lead implementation of care coordination activities for each Home and Community Care (HCC) service line including Hospice and Home Health , within the geographic area of responsibility with the goal of furthering the value-based activities of the ministries and the JV. This role will have a supporting relationship with local HCC ministry leaders and will be responsible for implementing a collective market strategy via engagement of hospital and JV leaders and through a comprehensive understanding of their operational considerations and goals.
