Reablement Assistant Practitioner | 10550 | Permanent Contract | 37 hours per week | £32,061 to £33,699 per annum | (Grade F) | Elizabeth House, Dereham Would you like to work in the community as part of our well-trained team of Occupational Therapists and support workers, creating person-centred personal plans helping people set and achieve their goals following an illness? Then this role is for you. We have opportunities available to visit people in their own homes in Thetford and the surrounding area to assess their social care needs. You may also work across other areas of South Norfolk depending on the needs of the service. Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence as much as possible while also reviewing any ongoing support needed.
